20and%20contributing%20factors%20for%20metabolic%20acidosis%20after%20deep%20hypothermic%20circulatory%20arrest%20%28DHCA%29.%5CnDESIGN%3A%20Retrospective%20observational%20study.%5CnSETTING%3A%20University%20hospital.%5CnPATIENTS%3A%20Eighty-seven%20consecutive%20patients%20undergoing%20elective%20thoracic%20aortic%20surgery%20with%20DHCA.%5CnINTERVENTIONS%3A%20Regression%20analysis%20was%20used%20to%20test%20for%20relationships%20between%20the%20severity%20of%20metabolic%20acidosis%20and%20clinical%20and%20laboratory%20variables.%5CnMEASUREMENTS%20AND%20MAIN%20RESULTS%3A%20Minimum%20pH%20averaged%207.27%5Cu00b10.06%2C%20with%2076%20%2887%25%29%20having%20a%20pH%26lt%3B7.35%3B%2055%20%2863%25%29%2C%20a%20pH%26lt%3B7.30%3B%20and%207%20%288%25%29%2C%20a%20pH%26lt%3B7.20.%20The%20mean%20duration%20of%20metabolic%20acidosis%20was%207.9%5Cu00b15.0%20hours%20%28range%3A%200.0%20-%2026.8%29%2C%20and%20time%20to%20minimum%20pH%20after%20DHCA%20was%204.3%5Cu00b12.0%20hours%20%281.0%20-%2010.0%20hours%29.%20Hyperchloremia%20contributed%20to%20metabolic%20acidosis%20in%2089%25%20of%20patients.%20The%20severity%20of%20metabolic%20acidosis%20correlated%20with%20maximum%20lactate%20%28p%26lt%3B0.0001%29%20and%20hospital%20length%20of%20stay%20%28LOS%29%20%28r%20%3D%200.22%2C%20p%26lt%3B0.05%29%2C%20but%20not%20with%20DHCA%20time%2C%20DHCA%20temperature%2C%20duration%20of%20vasoactive%20infusions%2C%20or%20ICU%20LOS.%20Patient%20BMI%20was%20the%20sole%20preoperative%20predictor%20of%20the%20severity%20of%20postoperative%20metabolic%20acidosis.%5CnLIMITATIONS%3A%20This%20retrospective%20analysis%20involved%20short-term%20clinical%20outcomes%20related%20to%20pH%20severity%20and%20duration%2C%20which%20indirectly%20may%20have%20included%20the%20impact%20of%20sodium%20bicarbonate%20administration.%5CnCONCLUSIONS%3A%20Metabolic%20acidosis%20was%20common%20and%20severe%20after%20DHCA%20and%20was%20attributed%20to%20both%20lactic%20and%20hyperchloremic%20acidosis.%20DHCA%20duration%20and%20temperature%20had%20little%20impact%20on%20the%20severity%20of%20metabolic%20acidosis.%20The%20severity%20of%20metabolic%20acidosis%20was%20best%20predicted%20by%20the%20BMI%20and%20had%20minimal%20effects%20on%20short-term%20outcomes.%20Preventing%20hyperchloremic%20acidosis%20has%20the%20potential%20to%20decrease%20the%20severity%20of%20metabolic%20acidosis%20after%20DHCA.%22%2C%22date%22%3A%222015-12%22%2C%22language%22%3A%22eng%22%2C%22DOI%22%3A%2210.1053%5C%2Fj.jvca.2015.07.025%22%2C%22ISSN%22%3A%221532-8422%22%2C%22url%22%3A%22%22%2C%22collections%22%3A%5B%22MJFIM5V6%22%5D%2C%22dateModified%22%3A%222022-03-02T19%3A48%3A27Z%22%7D%7D%5D%7D
