heng%22%7D%5D%2C%22abstractNote%22%3A%22BACKGROUND%3A%20Right%20atrial%20reentrant%20tachycardia%20resulting%20from%20lower%20loop%20reentry%20%28LLR%29%20around%20the%20inferior%20vena%20cava%20%28IVC%29%20has%20been%20described%20recently.%20However%2C%20all%20reported%20cases%20of%20LLR%20in%20the%20literature%20have%20negative%20flutter%20waves%20on%20the%20inferior%20surface%20ECG%20leads%20similar%20to%20that%20of%20counterclockwise%20typical%20atrial%20flutter%20around%20the%20tricuspid%20annulus%20%28TA%29.%20Right%20atrial%20flutter%20with%20positive%20flutter%20waves%20in%20the%20inferior%20ECG%20leads%20has%20been%20assumed%20to%20rotate%20as%20a%20single%20reentrant%20activation%20wave%20front%20around%20the%20TA%2C%20and%20the%20role%20of%20LLR%20in%20those%20patients%20is%20not%20known.%5CnMETHODS%20AND%20RESULTS%3A%20Twelve%20consecutive%20patients%20with%20flutter%20wave%20morphology%20on%20surface%20ECG%20consistent%20with%20clockwise%20atrial%20flutter%20were%20studied.%20The%20endocardial%20activation%20pattern%20recorded%20from%20conventional%20multipolar%20electrode%20catheters%20was%20characteristic%20of%20clockwise%20atrial%20flutter%20around%20the%20TA.%20Entrainment%20pacing%20in%20all%2012%20patients%20and%203D%20activation%20sequence%20mapping%20in%207%20patients%2C%20however%2C%20revealed%20clockwise%20LLR%20involving%20the%20lower%20right%20atrium%20around%20the%20IVC%20in%207%20patients%2C%20figure-of-8%20double-loop%20reentry%20around%20both%20the%20IVC%20and%20TA%20in%204%2C%20and%20single%20reentrant%20loop%20around%20the%20TA%20in%201.%20Linear%20radiofrequency%20ablation%20in%20the%20isthmus%20between%20the%20TA%20and%20IVC%20%28TI%20isthmus%29%20terminated%20the%20tachycardia%20in%20all%20patients.%5CnCONCLUSIONS%3A%20Surface%20ECG%20flutter%20wave%20morphology%20and%20limited%20recording%20intracardiac%20sites%20proved%20insufficient%20to%20delineate%20the%20precise%20mechanism%20of%20the%20TI%20isthmus-dependent%20clockwise%20right%20atrial%20flutters.%20Most%20right%20atrial%20flutters%20with%20positive%20flutter%20wave%20on%20surface%20ECG%20may%20be%20supported%20by%20a%20reentrant%20circuit%20around%20the%20IVC%20or%20a%20figure-of-8%20double-loop%20reentry%20involving%20both%20the%20IVC%20and%20TA.%22%2C%22date%22%3A%222004-04-06%22%2C%22language%22%3A%22eng%22%2C%22DOI%22%3A%2210.1161%5C%2F01.CIR.0000124221.84399.48%22%2C%22ISSN%22%3A%221524-4539%22%2C%22url%22%3A%22%22%2C%22collections%22%3A%5B%228EPJLFD3%22%2C%22DJYFUK6T%22%2C%22JZZF2YYQ%22%5D%2C%22dateModified%22%3A%222022-02-25T19%3A17%3A08Z%22%7D%7D%5D%7D
