me%22%3A%22Justin%22%2C%22lastName%22%3A%22Georgekutty%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Amir%22%2C%22lastName%22%3A%22Kazerouninia%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22YunFei%22%2C%22lastName%22%3A%22Wang%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Peter%20R.%22%2C%22lastName%22%3A%22Ermis%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Dhaval%20R.%22%2C%22lastName%22%3A%22Parekh%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Wayne%20J.%22%2C%22lastName%22%3A%22Franklin%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Wilson%20W.%22%2C%22lastName%22%3A%22Lam%22%7D%5D%2C%22abstractNote%22%3A%22OBJECTIVE%3A%20Adult%20Fontan%20patients%20are%20at%20increased%20risk%20for%20thrombosis%20and%20thromboembolic%20complications%20leading%20to%20increased%20morbidity%20and%20mortality.%20Most%20are%20prescribed%20antiplatelet%20or%20anticoagulant%20therapy%20for%20thromboprophylaxis%3B%20novel%20oral%20anticoagulants%20%28NOACs%29%20are%20uncommonly%20used%20given%20lack%20of%20data%20on%20their%20use%20in%20this%20population%20and%20generalized%20concerns%20regarding%20Fontan%20patients%26%23039%3B%20abnormal%20coagulation.%20We%20report%20the%20largest%20single-center%20experience%20with%20the%20use%20of%20NOACs%20for%20treatment%20and%20prophylaxis%20of%20thrombosis%20and%20thromboembolism%20in%20adult%20Fontan%20patients.%5CnRESULTS%3A%20A%20retrospective%20chart%20review%20identified%2021%20patients%20%2811%20female%2C%2010%20male%29%2C%20median%20age%2033%20years%20%2818-50%29%20at%20first%20initiation%2C%20who%20were%20prescribed%20a%20NOAC%20on%2027%20different%20occasions.%20The%20main%20indications%20for%20anticoagulation%20were%20arrhythmia%20%28N%5Cu2009%3D%5Cu200912%29%2C%20thrombosis%20%28N%5Cu2009%3D%5Cu20098%29%2C%20and%20persistent%20right%20to%20left%20shunts%20%28N%5Cu2009%3D%5Cu20092%29%3B%20one%20patient%20was%20initially%20on%20anticoagulation%20for%20arrhythmia%20but%20restarted%20for%20thrombosis.%20The%20most%20common%20indications%20for%20initiation%20of%20a%20NOAC%20over%20warfarin%20were%20patient%5C%2Fprovider%20preference%20%28N%5Cu2009%3D%5Cu200911%29%2C%20labile%20international%20normalized%20ratio%20%28INR%29%20%28N%5Cu2009%3D%5Cu20095%29%2C%20initiation%20of%20therapy%20elsewhere%20%28N%5Cu2009%3D%5Cu20093%29%2C%20and%20history%20of%20poor%20clinical%20follow-up%20%28N%5Cu2009%3D%5Cu20092%29.%20Over%20a%20cumulative%20316%20months%20of%20patient%20therapy%2C%20one%20new%20thrombotic%20event%20was%20noted.%20No%20major%20or%20nonmajor%20bleeding%20events%20occurred%2C%20and%2010%20patients%20experienced%20minor%20bleeding%20that%20did%20not%20require%20the%20cessation%20of%20therapy.%20One%20patient%20died%20from%20multiorgan%20system%20failure%20following%20an%20unwitnessed%2C%20out%20of%20hospital%20arrest.%20At%20present%2C%2010%20patients%20remain%20on%20NOAC%20therapy%20in%20the%20setting%20of%20ongoing%20arrhythmia%20%28N%5Cu2009%3D%5Cu20094%29%2C%20history%20of%20stroke%20%28N%5Cu2009%3D%5Cu20092%29%2C%20history%20of%20pulmonary%20embolism%20%28N%5Cu2009%3D%5Cu20092%29%2C%20history%20of%20deep%20vein%20thrombosis%20%28N%5Cu2009%3D%5Cu20091%29%2C%20and%20history%20of%20right%20ventricle%20thrombus%20%28N%5Cu2009%3D%5Cu20091%29.%5CnCONCLUSIONS%3A%20While%20our%20study%20is%20limited%20by%20size%2C%20our%20results%20suggest%20that%20NOACs%20may%20be%20a%20non-inferior%20alternative%20to%20traditional%20anticoagulation%20and%20that%20further%20study%20is%20warranted.%22%2C%22date%22%3A%22Jul%202018%22%2C%22language%22%3A%22eng%22%2C%22DOI%22%3A%2210.1111%5C%2Fchd.12603%22%2C%22ISSN%22%3A%221747-0803%22%2C%22url%22%3A%22%22%2C%22collections%22%3A%5B%22GW95W9I8%22%2C%22K6JKEATQ%22%2C%225P97GH9U%22%2C%22QNJADXH4%22%5D%2C%22dateModified%22%3A%222019-01-03T17%3A01%3A43Z%22%7D%7D%5D%7D
