torType%22%3A%22author%22%2C%22firstName%22%3A%22Steven%22%2C%22lastName%22%3A%22Gordon%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Gosta%22%2C%22lastName%22%3A%22Pettersson%22%7D%2C%7B%22creatorType%22%3A%22author%22%2C%22firstName%22%3A%22Michael%20A.%22%2C%22lastName%22%3A%22Bolen%22%7D%5D%2C%22abstractNote%22%3A%22Background%3A%20Significant%20improvement%20of%20computed%20tomography%20%28CT%29%20technology%20in%20the%20last%20decade%20has%20led%20to%20more%20use%20of%20this%20modality%20for%20evaluating%20infective%20endocarditis%20%28IE%29%20especially%20since%20the%20introduction%20of%20high%20resolution%20electrocardiogram%20%28ECG%29%20synchronized%20multiphasic%20%284D%29%20acquisition.%20While%20there%20are%20a%20number%20of%20reports%20on%20the%20accuracy%20and%20value%20of%204D%20CT%20for%20evaluation%20of%20IE%2C%20there%20is%20no%20published%20data%20regarding%20the%20performance%20of%20single-phase%20ECG%20gated%20CT%20for%20assessment%20of%20IE.%20The%20purpose%20of%20this%20study%20is%20to%20evaluate%20the%20sensitivity%20and%20specificity%20of%20preoperative%20single-phase%20ECG-gated%20CT%20imaging%20versus%20transesophageal%20echocardiography%20%28TEE%29%20in%20the%20assessment%20of%20complications%20related%20to%20IE%2C%20with%20comparison%20to%20surgical%20findings.%5CnMethods%3A%20Among%20899%20patients%20with%20surgically%20proven%20IE%20in%20our%20database%2C%20122%20underwent%20contrast-enhanced%20ECG%20cardiac%20CT%20and%20were%20included%20in%20the%20study%3B%2084%20of%20these%20patients%20also%20underwent%20TEE.%5CnResults%3A%20Overall%2C%20there%20was%20no%20significant%20difference%20between%20CT%20and%20TEE%20in%20the%20identification%20of%20pseudoaneurysm%5C%2Fabscess%20and%20dehiscence.%20For%20the%20detection%20of%20pseudoaneurysm%5C%2Fabscess%20in%20prosthetic%20valves%2C%20CT%20demonstrated%20higher%20sensitivity%20%2881%25%20vs.%2064%25%29%20and%20specificity%20%2875%25%20vs.%2033%25%29%20in%20patients%20with%20mechanical%20aortic%20valves%3B%20TEE%20demonstrated%20marginally%20higher%20sensitivity%20%2872%25%20vs.%2063%25%29%20and%20specificity%20%2880%25%20vs.%2073%25%29%20in%20patients%20with%20bioprosthetic%20aortic%20valves%2C%20although%20the%20differences%20are%20not%20statistically%20significant.%20TEE%20demonstrated%20significantly%20higher%20sensitivity%20%2885%25%20vs.%2016%25%29%20in%20identifying%20vegetation%20in%20all%20patients%20%28P%26lt%3B0.0001%29%2C%20including%20patients%20with%20prosthetic%20valves%20%28sensitivity%2C%2078%25%20vs.%2019%25%29.%20The%20combined%20imaging%20findings%20of%20CT%20and%20TEE%20demonstrated%20improved%20sensitivity%20in%20identifying%20pseudoaneurysm%5C%2Fabscess%20and%20slightly%20improved%20detection%20of%20prosthesis%20dehiscence.%5CnConclusions%3A%20Preoperative%20single-phase%20gated%20CT%20can%20be%20seen%20as%20complementary%20to%20TEE%20in%20assessing%20complications%20of%20suspected%20IE%20or%20may%20be%20substituted%20for%20TEE%20when%20vegetation%20or%20dehiscence%20is%20depicted%20on%20transthoracic%20echocardiography%20and%20the%20patient%20has%20a%20contraindication%20to%20TEE.%22%2C%22date%22%3A%222018-08%22%2C%22language%22%3A%22eng%22%2C%22DOI%22%3A%2210.21037%5C%2Fcdt.2018.07.07%22%2C%22ISSN%22%3A%222223-3652%22%2C%22url%22%3A%22%22%2C%22collections%22%3A%5B%22MZXU6P62%22%5D%2C%22dateModified%22%3A%222022-02-04T22%3A03%3A08Z%22%7D%7D%5D%7D
