CME Information - Overview

This presentation provides an overview of reducing the need for re-hospitalization in heart failure which is a major public health concern. To-date, no singular therapeutic intervention has been demonstrated to reduce re-hospitalization. However, a comprehensive evidence-based process of care improvement strategy has been demonstrated to favorably impact re-hospitalizations for heart failure. 

Educational Objectives
The intended result of this activity is increased knowledge. At the conclusion of this presentation, participants should be able to-

  • Address re-hospitalization in Heart Failure
  • Apply a "real" word application of best practices in Heart Failure
  • Develop a usable template to improve hospitalization outcomes in Heart Failure

Target Audience
The intended audience for this continuing medical education activity includes cardiologists, cardiovascular surgeons, and physicians.

Method of Instruction / Participation
This online enduring material comprises slide sets with text and audio derived from a live cardiology grand rounds presentation with post activity assessment and evaluation.
